TCDA Action Form
The TCDA has introduced the TCDA Action Form.
This form is intended to streamline communication between TCDA
members and their elected officials. Any TCDA member may
submit a TCDA Action Form. To use the Action Form, simply
complete whatever is applicable from line 1 - 14 and mail the
form to the address shown on the lower right corner of the form.
The League Secretary will provide your Action Form to the person
or persons best sited to deal with your suggestion, complaint, or
other submission. Please remember that this form is not intended
to be used to file a match protest. The procedure for filing
a match protest is in the Rules and Regulations you have been
provided with as a captain and on the TCDA Website on
the League Info page at the bottom.
There are two versions of the Action Form available. A PDF version and a
Form Fill Word doc version. Please select the version you wish to use.
